Cygnet Sherwood Lodge is a 17 bed specialist rehabilitation service for men with learning disabilities and other complex needs. The hospital also has nine transitional beds on site. Sherwood Lodge was rated as ‘Outstanding’ following its last CQC inspection.
The two new services are the two-bed Adele Cottage, just next to Cygnet Sherwood Lodge, and the Sycamores Bungalows located in Alfreton.
In line with Transforming Care, the aim of the new services is to support individuals with learning disabilities and complex needs to move out of hospital and transition back to community living, close to home.
The service model aims to achieve the following objectives:
- Reduction in hospital lengths of stay
- Relapse prevention – reduce re admissions and stop the “revolving door”
- Achieving successful discharge to independent living
- Maintenance and enhancement of living skills, and support in development of coping strategies
In order to encourage each individual to maintain autonomy and independence as far as possible, individuals are not excluded from undertaking activities simply because there is an element of risk involved. Positive risk taking is considered an integral part of living.
